Astarion often approves of self-interest, wit and power, but a successful romance does not require a murder-hobo route. Respecting his autonomy, taking his abuse seriously and helping him against Cazador matter just as much to the long arc. Before Save the Refugees resolves, reaching 40+ approval can make him propose a night together in normal dialogue. The tiefling or goblin celebration is the other major Act 1 start window when relationship slots permit it.
Roadmap: the Act 2 confession is the real lock-in

Act 1: you do not need to play evil
Bite Night is a separate camp-event chain. It is not triggered by finding the exsanguinated boar — that discovery merely adds dialogue. Letting Astarion bite can grant approval and context, but the romance can still start without making “let him bite” a mandatory condition.
The first intimate scene establishes Dating; the long-term relationship is not secured until an Act 2 confession.
Act 2 has two confession routes

Both scenes reveal the same core truth from different angles: Astarion started the relationship as a survival strategy, then realised the attachment became real. The confession is the point where the game moves him into the Partnered stage. Choosing “you need a friend more than a lover” ends the romance without destroying the companion relationship.
Act 2 partnering is built around Astarion’s autonomy and the question of whether the relationship is something real rather than transactional.
The Araj scene is a boundary test, not a potion min-max event
Araj Oblodra offers a mechanically tempting reward for having Astarion bite her, but he clearly does not want to. The romance route cares about what the player does with that refusal. Supporting him is compatible with the confession route; coercing him and then pressuring him into further intimacy is one way Astarion can end the relationship. Treat the scene as a character choice first and an item-reward choice second.
Act 3: Cazador and the Rite have party-level consequences
After Cazador is defeated, companion Astarion cannot complete the Rite without the player’s help. If he asks to ascend, there are several distinct outcomes. Convincing him not to ascend requires DC 18 Persuasion, or DC 15 Persuasion after a successful DC 20 Insight check reveals what is driving him. Succeeding keeps him in the party as a spawn. Simply refusing to help — or failing the persuasion route — makes him leave permanently. Starting the ritual and interrupting it turns him hostile.
The Cazador ritual is not only a romance choice: mishandling it can make Astarion leave the party or turn hostile.
Spawn romance: graveyard, autonomy and a new beginning
If Astarion is convinced not to ascend and the romance is partnered, the Act 3 conclusion takes place at his old grave. He treats the defeat of Cazador as a new birth, adds a new date to the tombstone and asks the player to remain in his future. Choosing sex or no sex during the scene does not end the relationship. The route’s theme is deliberately the opposite of Cazador’s control: a relationship he now chooses rather than performs to survive.
The spawn-route romance payoff is Astarion’s grave, where he reframes the end of Cazador’s control as a new beginning.
Spawn Astarion’s post-Cazador romance remains intimate without requiring the player to accept a new master/spawn hierarchy.
Ascendant romance: continuing means accepting a different power structure
If you help Astarion complete the Rite, he becomes the Vampire Ascendant and the tone changes sharply. To continue the normal romance path through the next long-rest conclusion, the player accepts becoming his vampire spawn. Refusing that transformation ends the relationship, though he normally remains in the party. The game treats this as a different relationship dynamic, not “the same romance with stronger combat stats.”

Safe workflow
1. Recruit Astarion early and long rest enough to see his camp events.
2. Reach 40+ before Save the Refugees resolves or use the celebration to start the Act 1 romance.
3. In Act 2 choose either Araj or the Yurgir/Raphael confession route; do not leave the act until the Partnered scene has played.
4. Respect bodily autonomy in Araj and intimacy follow-ups.
5. Save before Cazador if you want to compare spawn and Ascendant outcomes.
6. For spawn, use the persuasion route rather than flat refusal. For Ascendant, understand that continuing romance means accepting transformation into his spawn.
